Because Dietary supplements are available in health stores without prescriptions most consumers believe they are safe. But Some Body Building, Weight Loss and Sexual Enhancement dietary supplements contains hidden ingredients that can cause serious personal injury and sometimes lead to death.

Last week after 56 six cases of liver injuries and one death case were linked to the use of OxyElite Pro Dietary Supplements, the manufacturer USPlabs recalled all products containing Aegeline and VERSA -1, two illegal ingredients. (for more see USA Today)

The production of the sport supplement “Craze” was suspended by the manufacturer Driven Sports at the end of last month after tests conducted by the Harvard Medical School revealed the supplement contains potentially dangerous amounts of methamphetamine analog. The owner of Driven Sports, Matt Cahill was convicted in 2005 of felony charges for the sale of the weight-loss pills made from a highly toxic chemical pesticide banned from human consumption. Many adverse drug events are preventable and constitute Medical Malpractice. An adverse drug event occurs when a patient suffers injury resulting from medication use. Adverse drug events are the results of medication errors or of known side effects that may happen even if the medication is taken correctly.

According to a recent report from the Healthcare Cost and Utilization Project (HCUP) and led by Audrey J. Weiss, Ph.D. and Anne Elixhauser, Ph.D. , 380,000 to 450,000 hospitalized patients suffer preventable adverse drug events every year.

According to the most recent statistics, in 2011, the most common causes of ADE during hospital stays were Steroids, Antibiotics, Opiates, Narcotics and Anticoagulants with 8 out of 1000 adults over 65 experiencing one of them while hospitalized.

The MTA is rarely interested in settling man-under cases before trial and if it loses it will keep fighting through appeals. Out of the 92 lawsuits filed in the last five years, the MTA won 73 of them. For nine cases the payments were on average $40,000. Only five big cases resulted in total payouts of $33 million.

An unidentified motorist died in a car accident early Sunday at W. 240th St and Broadway in the Bronx, New York. His car flipped and caught fire after he hit one of the poles of the elevated subway.

Two people died and three others suffered personal injury in a 3 vehicle accident that happened Saturday morning in New York.

Atha Noorzi, a 51 year old limo car driver was driving his client Patrick Sheehan, a 54 year old Wells Fargo Banker from JFK airport to Larchmont, NY when the 45 year old driver of a black SUV driving in the opposite direction lost control of his vehicle. The black SUV hit the guardrail and went airborne into the opposite lanes. It then crashed into the Lincoln Town Car livery cab, slicing in half and killing the two passengers. Sheehan was decapitated.

A white SUV also crashed. The woman who was driving it and her 15 year old passenger as well as the driver of the black SUV suffered injuries and were taken to the hospital.

Seven car accidents and two injuries have been caused by a defective tie-rod assembly that became misaligned in some Chrysler Ram Trucks. The misalignment can lead to component fracture and steering loss. These accidents prompted a recall by Chrysler that will include an estimated 842,400 model-year 2003-2008 Ram 2500 and 3500 trucks. Two additional campaigns are being conducted to capture those vehicles known to have tie-rod assemblies linked to previous recalls. They comprise 294,000 model-year 2008-2012 Ram 2500 and 3500 pickups, chassis cabs and 2008 Ram 1500 4×4 Mega Cabs; plus 43,500 model-year 2008-2012 Ram 4500 and 5500 4×4 chassis cabs.

Retained%20Surgical%20Items.jpgThe unintended retention of foreign objects (URFOs) is medical malpractice that can cause serious personal injury or death. It happens when a surgical team leaves any item or foreign object related to the surgery inside a patient. Most common objects left behind are sponges and towels, small device components or fragments, needles and malleable retractors. The risk of URFOs is higher for patients with high body mass index, during emergency procedures or when an unanticipated change happens during the surgery.

In a recent sentinel event alert The Joint Commission looked at the causes of these surgical errors and recommend strategies for improvement.

According to the Commission previous studies show that the risk of URFOs can be greatly reduced by the creation and the adoption by the surgical team of a highly reliable and standardized counting system. Effective communication including team briefings and debriefings as well as appropriate documentation and safe technology are also factors that can reduce this type of medical malpractice.

The train accident in which Two Metro North commuter trains serving New York City collided last May could have been avoided. Robert Puciloski, the Metro-North chief engineer admitted that poor track inspection and poor maintenance led to the accident. He was testifying yesterday before the National Transportation Safety Board (NTSB) investigative hearing in Washington.

Six weeks before the accident, inspectors discovered that a pair of joint bars connecting two sections of rails were cracked. The bars were replaced but two days before the accident inspectors reported a “pumping condition” meaning the ties and rails were moving up and down under the weight of trains, and “inadequate ballast support”.

The report was vague and there were no measures of the pumping conditions. The inspectors said they were pressured to work quickly so they would not interfere with the regular flow of trains.

More than 3000 people die every year in distracted driving accidents but some reckless drivers don’t seem to care. The new “trendy” thing to do is to post Driving Selfies on Instagram and other social media sites. This afternoon research for #drivingselfie on Instagram returned 3,774 posts, #drivingselfies 1,886 posts #drivingtowork 9,826 posts and #hopeidontcrash 730 posts.

These reckless drivers are unnecessarily risking their lives and the lives of innocent people. Driving is not a game.

Another pedestrian was killed in a car accident in New York. A 74 year old woman was in a crosswalk on E. 141st street in South Bronx when she was hit by an SUV heading South on Beekman Street. She suffered a severe head injury and died at the hospital.

After striking the pedestrian, the SUV jumped the curb and ran into a building. The driver, a 47 year old woman and her passenger, an 8 year old girl suffered minor injuries.
