A nurse who committed a medical error and injured a nursing home patient by administrating morphine instead of a muscle relaxant and then tried to cover her error by falsifying documents was indicted by New York Attorney General Eric T. Schneiderman

A wheelchair-bound resident of a Long Island nursing home suffered serious personal injury after a nurse committed a medical error and injected him with morphine instead of a prescribed muscle relaxant and then attempted to cover up her error by falsifying documents. The nursing home resident overdosed and had to be admitted to a hospital where Narcan, a medication to counter the effects of an opiate overdose was administered. The nurse, Vicki Price, was charged recently with one count of endangering the welfare of a vulnerable elderly person, or an incompetent or physically disabled person, in the second degree, a class E felony; one count of endangering the welfare of an incompetent or physically disabled person, a class A misdemeanor; one count of willful violation of the public health laws, an unclassified misdemeanor; and two counts of falsifying business records in the first degree, a class E felony. She faces up to 1 1/3 to 4 years in prison if convicted.

9 children and two adult injured in school bus accident in Queens, NYC

11 people suffered non threatening personal injury after a car struck a school bus that was stopped at a red light at the intersection of Liberty Ave and 183rd Street in Jamaica, Queens, New York on Tuesday.

9 children and two adults were injured and transported to the hospital; two of them suffered serious personal injury.

This school bus accident is the second one since classes started again on September 4th. Last week nearly 30 children were injured when two school buses collided in the Bronx, NYC.

1 dead and 2 injured in an accident between a truck and a minivan in the Bronx, NYC

An unlicensed driver suffered personal injury and his passenger died after he crashed the minivan he was driving into the back of a truck. The accident happened early Tuesday morning at Tiffany Street and Barry Avenue in Hunts Point in the Bronx, NYC. The driver was arrested and charged with driving without a license. According to CBS New York, the minivan involved in the crash was stolen and the driver was also charged with grand larceny of an auto and criminal possession of stolen property.

OSHA now requires employers to report employee fatalitities within 8 hours and severe injury requiring hospitalization, amputation or loss of an eye within 24 hours

OSHA recently updated its Injury and Illness Recording and Reporting regulation by announcing a new rule that requires employers to report not only single fatalities but also single hospitalizations, amputations or loss of an eye. Previously, OSHA’s regulations required an employer to report only work-related fatalities and in-patient hospitalizations of three or more employees.

Additionally OSHA also updated the list of industries that are partially exempted from this rule due to relatively low occupational injury and illness rates.

Minivan taxi driver fatally strikes a man who was changing a tire on the Van Wyck Expressway and injures 3 other people

A man died and two women and a child suffered personal injury in a car accident on the Van Wyck Expressway near JKF Airport. According to the New York Daily News, the victim had pulled his car to the shoulder of the highway to change a flat tire when he was hit by a minivan taxi registered with the Taxi and Limousine Commission Sunday around 4:00 am. A woman who was in the victim’s car and another woman and a child who were in the taxi suffered personal injury and were taken to the hospital.

A 32 year old woman suffered head trauma and a 27 year old man was left critically injured with both legs amputated by a hit and run driver in Brooklyn NY

A SUV driver abandoned his car on the sidewalk after an accident during which he lost control of his vehicle, jumped the curb and pinned two pedestrians against the wall of a building in Starrett City, Brooklyn, NYC. One of the pedestrians, a 27 year old man was severely injured and had both legs amputated. The other pedestrian, a 32 year old woman suffered head trauma but she is in stable condition.

Five people injured and one dead in a multi-car accident in Rosedale, NYC

Five people were sent to the hospital and a 24 year old motorcyclist died in a car crash involving several vehicles near the intersection of Rockaway and Brookville in Queens, NYC. The motorcyclist was riding his bike on Rocakaway Blvd when it broke down. Several people met him and helped him load the bike on a pick up truck. As they were doing so a Chevy van hit the back of the pick up truck and struck them. Another car then hit the Chevy van. The motorcyclist died at the scene and five other people suffered personal injury and were rushed to the hospital.

Deadly car accident on the Bronx River Parkway on Friday night was most probably related to speeding

Two men died and a third one suffered critical personal injury in a car accident that happened last Friday on the Bronx River Parkway in New York around 4:00 am. According to witnesses, the victims were in a blue Mini Cooper and seemed to be racing another car when the 20 year old driver, Xavier Rodriguez, lost control of the vehicle. The car hit a tree and and was sheared in two before bursting into flames. The driver and the front seat passenger were ejected from the car. They were rushed to the hospital in critical condition. The driver later died of his injuries. A third passenger, 24 year old Bill Bido, who was in the back seat was badly burned and died at the scene of the crash.

Senator Schumer secures $25 million in federal funding for NYC Vision Zero Initiatives to reduce traffic fatalities and injuries

13 projects to prevent pedestrian and bicycle accidents in New York City are going to be funded by a $25 million federal grant secured by Senator Schumer. Previous Vision Zero Crash analysis and input from communities were used to select the13 project locations. Projects will include an extension of the Brooklyn Waterfront Greenway; street safety improvements near schools in Manhattan, Queens and Staten Island; and pedestrian safety measures along dangerous corridors like Fourth Avenue in Sunset Park.

73 year old New York pedestrian suffers critical personal injury after being struck by 87 year old driver on the Upper West Side

Another pedestrian was hit by a car on West End Avenue. The victim a 73 year old woman is in critical condition. She was crossing West End Ave yesterday night around 8 pm when an auto driven by an 87 year old man ran over her. The driver stayed at the scene and wasn’t charged. (read more in the Gothamist)

West End is very a very dangerous avenue. This year, 61 year old Jean Chamber and 9 year old Cooper Stock were both killed in car accidents on West End Ave. Between 2008 and 2012, 96 pedestrians, 41 cyclists and 160 motor vehicle passengers were injured in crashes on West End. To stop the bleeding, The NYC Community Board 7 approved a complete redesign of West End Ave (see previous blog) that should be completed before the winter. (see West Side Rag)
