Two construction workers as well as several pedestrians and car passengers suffered personal injury in a crane accident in New York yesterday morning. The crane was hauling an air conditioning unit of the size of a shipping container to the top floor of a 30-story building at 261 Madison Avenue, N.Y.C. when a rigging strap broke. The unit first crashed into the side of the building before landing on Madison Avenue between 38th and 39th streets. 10 people were injured by the falling debris. The crane operating company, Skylift, had a similar accident in 2010 during which no one was injured and received two serious OSHA violations.

New York car accidentThe New York Daily News analyzed NYPD crash data from July 2012 to March 2015 to see if the Zero Vision Plan launched by Bill de Blasio was making the streets of New York City safer. According to the newspaper analysis that compared the road statistics after the Vision Zero Roll out from September to December 2014 with the same period of 2012, the results are mixed.  Some roads had very large decline in the numbers of vehicles accidents resulting in severe personal injuries or fatalities such as Broadway between 125th street to 95th Street in Manhattan but other roads saw an increase in accidents resulting in injuries or fatalities. For example during the months following a reduction of the speed limit on E. Gun Hill Road in the Bronx, car accidents on this road resulted in 50 fatalities or severe personal injuries while for the same period of 2012 this number was 33.

The daughter of Reverend Sharpton who sued the city for $5 million after injuring her ankle in Soho recently posted pictures of herself climbing mountains in Indonesia. She has also been pictured in high heels at a hippie party and climbing the dicey terrain of Red Rock Canyon in Nevada.

Greta GreeneA 2 year old girl who suffered severe personal injury yesterday after being hit by falling debris, died Today at the hospital. Greta Greene (facebook picture on the left) was sitting outside with her grand mother when they were both struck by terra-cotta bricks from a window ledge that fell from the 8th floor of  the Esplanade Manhattan nursing home at 305 West End Ave in Manhattan.

The toddler lost consciousness and nurses from the nursing home tried to resuscitate her. Her grandmother, 60 year old Susan Frierson, suffered severe leg injuries. They were both transported to the hospital where Greta passed away Today.

A protective scaffolding was erected last night on orders on the Department Of Buildings. The DOB is investigating what caused the ledge to fall. It also issued a violation to the Esplanade Manhattan nursing home for failure to maintain a property in a safe and compliant manner.

40 people died and  10,069 suffered personal injury in a traffic crash in New York City between January 1st and April 1st 2015 compared to respectively 51 and 10,805 in 2014 and 69 and 11,676 in 2013.  it is also interesting to note that there were more accidents during the three first months of this year compared to the same period of the 2 previous years. These numbers indicate that streets are getting a little safer for New Yorkers and that Vision Zero may be starting to show some results.

23 pedestrians died in traffic accidents in NYC during the first three months of the year compared to 31 for the first three months of 2014 and 44 for the same period of 2013. This number is still high but over the last two years it almost decreased by half which is a positive evolution.  The number of pedestrians injured in a crash in NYC during that same period was 2,423 in 2015 compared to 2,847 in 2014 and 3,080 in 2013.

1 person died in a bicycle accident in New York City during the first trimester of 2015 compared to 2 the previous year and 1 in 2013. 355 cyclists suffered personal injury during the same period compared to 399 in 2014 and 510 in 2013. It is difficult to look at bicycle accident statistics for the months of January, February and March as it is the end of the Winter season and not many people use their bikes.

6 people were injured in a car accident during which a livery van collided with another car on Park Ave at E. 116th Street in Harlem, NYC. The collision was so powerful that the livery car flipped into the air. Two people were seriously injured and four others suffered only minor injuries. They were all rushed to the hospital.

New York Truck accidentThankfully nobody was injured in a truck accident that happened Saturday at the site of  a new Memorial for Traffic Fatalities in Brooklyn, NYC. Activists for the group Right of Way were in the process of creating a huge mural depicting the silhouettes of the 264 people who died in traffic accidents in New York in 2014 when an 18-wheeler plowed into it.

Organizer Keegan Stephan was standing closest. The crash pulled parts of the plywood fence from its frame, and he said he and other volunteers who were wheat-pasting posters down the block had to jump back to avoid being struck. The driver could have done worse. “Fortunately he just hit a wall,” Stephan said. “If someone had been standing there that person would be dead.”

Two men suffered personal injury in a collision between a car and a motorcycle in the Bronx, NYC. The accident happened at the busy intersection of Baychester and Nereid. During the investigation the police discovered a bullet hole in the car which led them to suspect that the car driver may have ran a red light before the crash as he was fleeing the scene of a shooting that happened a few blocks away. Both the driver – who was shot – and the motorcycle driver were injured and rushed to the hospital.

3449 people were injured and 13 people died  in traffic accidents in New York City in January 2015 compared to respectively 3899 and 21 in January 2014 , 3993 and 28 in January 2013 and 4179 and 21 in January 2012. These numbers are encouraging and may indicate that the Vision Zero Program started approximately a year ago may be effective in making New York City streets safer. Data from December 2014 and January 2015 also show, for the first time, two consecutive months during which the total monthly number of traffic fatalities was below 14. The total number of motor vehicle accidents was stable. Below are graphs depicting traffic accident rates from January, 2013 to January 2015 for various types of accidents for each month for this time period in NYC.

6 pedestrians died in traffic accidents in January 2015 compared to 12 in January 2014, 20 in January 2013 and 12 in January 2012. This is the lowest number since May 2013. This is also half the number recorded the same month a year before.  In January 2015, 902 pedestrians suffered personal injury after being struck by a car compared to respectively 1075, 1107 and 1045 in 2014, 2013 and 2012. Its the first time that this number is below 1000 for the month of January.

115 people were injured and 1 person died in bicycle accidents in January.  Bike accidents are usually at their lowest in New York City during the months of January and February because less people use their bikes because of the bad weather and the snow.

Wallcure simulation ssalineTwo patients were seriously injured in a New York urgent care facility after they were inadvertently administered non sterile simulation intravenous fluids. They both experienced a febrile illness during administration and had to be hospitalized. One of them developed sepsis. Both of them survived.

The cases were reported to the New York State Department of Health (NYSDOH)last year. The NYSDOH  began a collaborative investigation with the CDC in December.  The investigation found that four other New York outpatient facilities had received Wallcur simulation saline. All facilities said they had ordered the real product and weren’t aware that they had received a simulation product until they were were notified by the NYSDOH. Fortunately none of the facilities had used the product yet.

Wallcur recalled all its saline simulation products from the market at the beginning of the year (see previous blog). Investigation was pursued by the CDC at a national level. So far nationally 9 adverse events have been reported for 25 people including 11 hospitalizations. Two deaths occurred even though it wasn’t clear that they were related to the administration of the product.  All clinical facilities that received the products confirmed that they were not aware at time of purchase that the product was intended for simulation only.
